Step-by-Step Female Diagnosis 

Initial Fertility Consultation

At FIV Valencia, we offer the opportunity to undergo an Individualised Diagnostic Study (EDI), allowing you to assess your fertility during your first visit. 

The initial consultation lasts one hour, during which your doctor will take the time to have an in-depth conversation with you, gathering vital information about your personal, family, and gynaecological history. This step is crucial in determining the most suitable treatment for you. 

Details such as the regularity and nature of your menstrual cycle, major illnesses or surgeries, allergies, and any current medications can all influence your fertility. A comprehensive evaluation helps your doctor plan any additional necessary tests or analyses tailored to you. 

During this visit, we will conduct some initial tests, such as an ultrasound and a blood test. 

Gynaecological Ultrasound in Fertility Diagnosis

A gynaecological ultrasound, carried out during your first visit, allows us to rule out any pathology and to carefully evaluate the size, shape, and receptivity of your uterus. We will measure the uterine cavity and endometrium (the uterus's inner lining). 

The ultrasound also enables your doctor to assess the condition of your ovaries and fallopian tubes. 

Blood Test: Anti-Müllerian Hormone

Measuring the blood levels of the anti-Müllerian hormone (AMH) provides information about your ovarian reserve, an indicator of your fertility. 

Women are born with a finite number of eggs, known as the ovarian reserve. Hormonal measurement helps determine how many eggs are available for Assisted Reproduction treatment using your own gametes. 

Your doctor may also request additional blood tests to evaluate your overall health, to detect infections, to assess hormonal balance, and to identify other factors that may contribute to infertility. 

Emotional Support and Counselling in Fertility

Fallopian Tube Permeability Test: Hysterosalpingo-Contrast Sonography (HyCoSy)

A common cause of female infertility is blocked fallopian tubes, which prevents the egg from reaching the uterus and thus hinders fertilisation. 

Fallopian tube obstruction often results from gynaecological infections, inflammation, adhesions, or conditions like endometriosis. Diagnosis is usually done via ultrasound or laparoscopy. 

When a tubal obstruction is identified, In Vitro Fertilisation (IVF) is the only option for achieving pregnancy with your own eggs. 

The Hysterosalpingo-Contrast Sonography (HyCoSy) procedure allows us to verify whether the fallopian tubes are open. A contrast agent is used to monitor tubal permeability through ultrasound. Unlike Hysterosalpingography (HSG), which uses X-rays, HyCoSy does not expose patients to radiation and is more comfortable. 

Genetic Testing in Assisted Reproduction

Genetic testing is recommended when the patient's medical history suggests that genetic factors may affect their ability to conceive. Genetic analyses are also conducted in cases of hereditary conditions or unexplained miscarriages. 

These tests help identify underlying genetic conditions influencing fertility, providing valuable insights for diagnosis and personalised fertility treatment planning. 

Diagnostic and Surgical Hysteroscopy

Hysteroscopy is a diagnostic procedure indicated during Assisted Reproduction treatment to examine the uterine cavity and to gain a clear view of its anatomical state. Performed via endoscopy, the doctor may take an endometrial sample for further histological analysis during the procedure. 

Hysteroscopy also enables the detection and immediate removal of any undesired pathology, such as polyps, fibroids, adhesions, or septa. 

This procedure is commonly performed under general anaesthesia to ensure patient comfort.

Tests After Multiple Failed IVF Attempts 

For complex cases or when previous Assisted Reproduction treatments have failed, more specific diagnostic tests may be prescribed, including: 

  • Vaginal and/or Endometrial Microbiota Study: To identify dysbiosis (changes in bacterial populations) and its potential inflammatory effect on the endometrium. This test involves an endometrial biopsy to rule out possible endometritis, which can cause repeated implantation failures or recurrent miscarriages. 
  • Haematology Study: Thrombophilia, a condition that increase blood clot formation, can interfere with pregnancy development. Blood tests or buccal swabs can help identify thrombophilia as a potential cause. 
  • Immunology Tests: The immune system may produce antibodies that mistakenly attack sperm, eggs, or embryos, leading to miscarriages or difficulties with IVF. FIV Valencia conducts reproductive immunology tests via blood samples to detect and address these conditions. 
  • ERA Test (Endometrial Receptivity Array): A genetic test determining the optimal window when the endometrium is most receptive to embryo implantation. This test benefits women who have experienced multiple unsuccessful embryo transfers by identifying the best timing for transfer.
